MTF Top is a transformative procedure and delivers long-lasting results, which typically last around 10-20 years, requiring minimal upkeep. Implants are designed for durability and patients’ comfort in their daily lives.

Choosing your implants is a key part of your MTF top surgery process in Fort Lauderdale. Our expert plastic surgeons at South Florida Center for Cosmetic Surgery will create a personalized approach to help you understand your options and choose a type of implant that aligns with your body, preferences, and gender-affirmation goals.
For MTF top surgery, you have the option to choose between silicone or saline implants. This gives you flexibility and variety to choose the best option that aligns with your unique anatomy and long-term aesthetic goals.
Both types of implants offer a wide variety of profiles and sizes. This helps your surgeon to deliver the best results to achieve a balanced, feminine, and natural-looking chest contour. You will have until the day of your surgery to make any final changes.
Patients have the option to either combine fat grafting with their implants or choose a more natural approach and do the fat grafting by itself. The fat used is typically taken from areas like the abdomen, flanks, or thighs, which is purified and carefully injected into the chest to create a fuller and more feminine contour.
This technique is ideal for those patients who are looking for a subtle enhancement without the need for implants and want to shape their chest only using their own body. During your consultation, you will be examined by your surgeon to assess your eligibility, body composition, and your goals.
You may choose between round or anatomical (teardrop-shaped) implants. Round implants create more upper pole fullness, while anatomical implants provide a more sloped, natural appearance. Your surgeon will help guide you based on your chest anatomy and desired outcome.
Profiles refer to how far the implant projects from the chest. Low, moderate, and high-profile options are available to fine-tune your silhouette and create the most proportionate result for your frame.
Implants can be placed either above the muscle (subglandular) or partially beneath the muscle (submuscular). In MTF top surgery, submuscular placement is often recommended to ensure a smooth, natural transition from the chest wall, particularly in patients with less native breast tissue.
Your surgeon will examine your skin elasticity, chest dimensions, and whether you have been in any prior hormone therapy to ensure optimal results. Our goal is to create results that not only look and feel natural but also feel empowering and affirming with your aesthetic vision.

30 - 1 Day Before Surgery
You will have your final consultation with your surgeon to answer any questions you may have. Our pre-operative team will ensure you complete your bloodwork, obtain any required medical clearance, and take before pictures to get you ready for surgery day.
Surgery Day
MTF Top surgery takes around 1.5 to 2.5 hours, and it is performed under general anesthesia. Your surgeon will make a small incision that allows him to insert the breast implant in the correct position. This is an outpatient procedure, and you’ll return home right after surgery wearing a post-surgical bra to support your new breast contour.
1 Day Post Op
Your surgeon will advise that you make rest your priority for the first 24-72 hours. Swelling, soreness, and tightness are expected. You must avoid any upper-body movement and continue wearing your post-surgical bra to support healing and circulation.
1 Week Post Op
You’ll return to our office one week post-op to meet with your surgeon, where your healing progress and incision areas will be monitored. Many patients feel comfortable and are cleared to return to light, non-strenuous activities. You must continue wearing your post surgery bra to optimize results.
1 Month Post Op
Swelling continues to subside, and your new breast shape begins to settle and to feel more comfortable. Many patients are cleared to return to light, non-strenuous exercises. You must continue wearing your post-surgical bra to make sure your final results are supported.
2 - 3 Months Post Op
Most patients feel fully healed during this stage and are now cleared to return to normal activities, including upper body movement and exercises. Your new breast contour will continue to soften and fall into position. Your small scar begins to fade, and some patients decide to only keep wearing their post-surgery bra at night.
6 - 12 Months Post Op
At this stage, this is when you’ll see your final results, your new breast contour is in its final position, and the implant now feels like natural breast tissue. Your scars keep healing and becoming invisible. Using proper aftercare will optimize the healing.

I had an amazing experience with Dr. Pinnella. He was so kind and easy to talk to. He chatted with me before surgery like I was a family member. Dr. Pinnella was very honest and knew exactly what was going to work for my body. He won’t do anything that he knows isn’t going to work for your body, nor will he do anything that he thinks won’t look good. This doctor definitely wants you to be happy with yourself after your procedure. I would choose him over and over again.

MTF Top surgery is a cosmetic, gender-affirming procedure designed to help transwomen to achieve a fuller breast structure with the use of implants to create a more feminine and natural chest appearance. This helps transgender women and nonbinary individuals achieve a fuller contour that aligns with their identity.
Ideal candidates are generally in good health, have realistic expectations, and desire a more feminine chest appearance. Whether or not you’ve undergone hormone therapy, your surgeon will assess your anatomy, skin elasticity, and goals during your consultation.
Hormone therapy is not required, but patients who have been on estrogen for at least 12 months may experience more natural breast tissue development, which can enhance the final result. Your surgeon will discuss your options based on your current medical transition status.
The most popular option for MTF Top surgery is silicone implants, which, over time, feel natural with a breast-like feeling and look. Patients can choose from a wide variety of shapes, sizes, and profiles.
MTF Top surgery takes around 1.5 to 2.5 hours, and it is done under general anesthesia to guarantee the patient’s comfort and safety. Our center performs outpatient procedures, and you’ll return home right after surgery wearing a supportive post-surgical bra.
The most common incision type is placed in the inframammary fold, beneath the breast. This placement allows your surgeon to insert the implant in the correct place, and it leaves a well-concealed scar. In some cases, you may opt for a periareolar incision.
Yes, any cosmetic surgery includes some type of scarring; however, MTF top surgery has very discreet and small incisions that will be placed on the natural fold of your new breast contour. With time, your scars will fade and become almost invisible as long as you matain a proper aftercare and scar management.
Your surgeon will advise you to prioritize rest and limited movement during the first 2 weeks. During this stage, you have to avoid upper body movements and wear a surgical bra that supports your healing and your new breast shape. Many patients feel fully healed and can return to normal activities within the 2-3 month mark.
Many patients notice an immediate improvement after surgery. You will be able to enjoy the change in volume since early on, swelling and implant settling take time. Patients can see their final results around the 3-6 month mark.
Yes, alongside your surgeon, you will be able to choose the size, profile, and placement that best fits your anatomy and structure. You will have time to make any changes or adjustments until the day of your surgery. We will work closely with you to find the best look that feels natural for your body.
In most MTF top surgeries, nipple-areola repositioning is not needed; this is more typical in breast lift cases where the areola has been impacted by stretched skin. Repositioning will also depend on your anatomy and the amount of skin present. Your surgeon will examine the best techniques for you.
Breast implants are considered long-lasting, but they are not made to last a lifetime. Most patients are advised to change their breast implants every 10-20 years, and even some people choose to change them earlier to avoid any risks of complications. Your surgeon might also let you know that if your implants are not causing any issues, changing them is not mandatory.
Yes, many patients choose to combine their MTF top surgery with other body contouring procedures to achieve a more complete transformation. The most common combinations are with liposuction or waist sculpting surgery. Your surgeon will customize your plan to fit your aesthetic goals.
At the South Florida Center, MTF top surgery starts at $1,999 (excluding anesthesia and surgeon fees). Your coordinator will provide your personalized final quote, which will vary depending on the complexity of your case and if you’re combining your MTF top surgery with any other procedures.
