The incision your surgeon chooses during breast augmentation directly affects scarring, implant placement, and your overall results. There are four primary incision types used in breast augmentation surgery: inframammary, periareolar, transaxillary, and transumbilical. Each has distinct advantages depending on your body, your implant choice (saline vs. silicone), and whether you may want a breast lift performed at the same time.
Inframammary incision is the most commonly used approach in breast augmentation. Your surgeon makes a small incision (typically 3–5 cm) in the natural fold beneath the breast. This gives the surgeon the most direct access to create the implant pocket, which allows precise placement of both silicone gel and saline implants. The resulting scar sits in the breast crease and is typically concealed by the breast itself. Periareolar incision is made along the lower border of the areola, where the darker skin meets the lighter breast skin. This natural color transition helps camouflage the scar. The periareolar approach is a strong choice if you are considering combining breast augmentation with a breast lift, since both procedures can be performed through the same incision site. This incision works best with smaller implants.
Transaxillary incision is placed in the natural crease of the armpit. The implant is inserted through a small tunnel created from the armpit to the breast pocket. This approach leaves no scarring on the breast itself. It is most commonly used with saline implants, which can be inserted deflated and then filled once in position. Some surgeons use endoscopic tools for greater precision with this technique.
This small incision is placed in the natural breast crease. This is the most common type of incision, which offers optimal access to the internal issue and offers a discreet scar that is hidden under the breast once the implant settles.
This incision is placed around the border of the areola (the pigmented skin around the nipple). This incision allows your surgeon to reach the precise area where the implant will be placed. Once the incision is healed, it blends with the natural color of the areola.
This incision is placed in the natural underarm fold. This technique doesn’t leave any scars around the breast area. However, a proper evaluation needs to happen because this technique is not appropriate for every patient or implant type.

Under the Muscle
This is the most common placement for breast implants. The implant is placed partially beneath the pectoral muscle. This type of placement achieves a more natural result and better implant coverage. This technique is beneficial for patients with thinner breast tissue. It may also reduce the risk of complications like rupture and capsular contracture and minimize visible rippling.
Over the Muscle
This implant placement sits right behind the breast tissue but on top of the muscle. Placing the implant over the muscle can offer a more dramatic round look, with quicker recovery. This option is good for patients with a good and healthy amount of natural breast tissue.
The dual plane technique is typically used for those patients with signs of mild sagging, improving their shape and contour. This advanced technique places the implant partly under the muscle and partly under the breast tissue, combining the benefits of both placements.

No. Silicone gel implants come pre-filled, so they require a slightly larger incision. The inframammary and periareolar approaches accommodate silicone implants well. The transaxillary approach can work for silicone in some cases, but TUBA (transumbilical) is only used with saline implants.
